Yet, the Joint Health Management Board (JHMB) believes that you shouldn’t find it difficult to locate the right healthcare provider for you and your family when you need one.
In partnership with its key vendors, the JHMB has created quick guides to help you locate network providers for your medical, dental, vision, behavioral health, and chiropractic care benefits. When contacting a provider, be sure to have your insurance card available–it contains plan information your provider will most likely need to answer your questions, including group number and network.

If you are enrolled in an Anthem Blue Cross medical plan, Delta Health Systems’ website is your online resource for finding doctors, hospitals and other healthcare professionals that participate in the Blue Cross Network. Visit www.deltahealthsystems.com to search for physicians by name or location.

Your Claremont Intake Counselor will do a brief assessment to help identify a provider that will fit your needs based on the presenting issue, your preferred locations/times, and any other personal preferences (i.e male/female, clinical orientation, any specific language needs, etc.). You will be referred to a provider that is also part of the Avante network in case you need further treatment covered under the Behavioral Health Benefits that Avante provides.
